Passed away peacefully at home surrounded by family on Sunday December 20, 2015 at the age of 55. Beloved husband of Jan. Proud father to Courtney and Cassandra. Cherished son of James and the late Phyllis. Adored brother to Elizabeth and Pauline. Sam will be sadly missed by his extended family, friends and all who knew him. Friends may call at MOUNT LAWN FUNERAL HOME AND CEMETERY 21 Garrard Road Whitby north of Dundas (905-443-3376) on Wednesday December 23, 2015 from 2-4 and 6-8 pm. Funeral sevice in the chapel on Thursday December 24, 2015 at 11 am. in lieu of flowers donations may be made to Lakeridge Health Foundation Cancer Care would be greatly appreciated by the family.</div>
